Request Forms:

• Clubs and Organizations Guide
  This is a guide for all clubs and organizations and can be used as a resource for advisors or club members.
  
• Provisional Club Application/Advisor Expectations
  This form is for new organizations and student clubs wishing to obtain a permanent status granting them the ability to request funding from the Student Senate.
• Student Life Budget Request and Annual Report
  This form is for existing clubs to secure funds for the next year and is due February 1st of each fiscal year. The senate will also review what your club accomplished the previous year.
• Special Circumstance
  This form is for all non club related activities and special budget requests for sanctioned clubs. Deadlines for this form are October 1st and February 1st of each fiscal year.
